PHYSIO SERVICES
We have a self referral service with MSK which you can ring yourselves to book an appointment without the need for a referral from your GP. Please call 01924 792075 to speak to MSK directly or 01924 402073 to speak to one of our reception team who will be happy to help.
IAPTs Mental Health Service
We offer a self referral to IAPTs for patients experiencing Mental health Issues that do not require urgent medical attention from the Crisis Team. Please call 01484 343700 to speak to IAPTs directly or 01924 402073 to speak to one of our reception team who will be happy to help.

Social Prescriber and Care Co-Ordinator Services
If you need advice or help around any social aspects or are a carer or are been cared for, we offer a service in surgery or as a Telephone call to discuss your needs. Please call our reception team on 01924 402073 to ask for more information or to book an appointment.
Diabetic Nurse Specialist
We have a DSN who works within our PCN to see our patients who have been referred from our practice nurse. We offer this service to help patients who cannot access a hospital appointment. This service is provided through our PCN. ( primary care network) We also have a Diabetes Dietitian service from Mid Yorks Hospital Trust who work with us one Tuesday in the month.
Maternity Medical Services
The midwife and GP will monitor the progress of confinements usually between 9 weeks to full term. Once pregnancy has been confirmed and appointment will be made to see the midwife who will then make any further appointments you need .She will also explain any benefits you may be entitled to during and after your pregnancy. Your appointment will be at Dewsbury Bronte Tower, Maternity Dept.
Post Natal and Baby 6-8 week check examination appointments will be sent to you by post or via telephone and are carried out by the GP. Please bring the babies red book along with you when attending.
Chaperone Policy
Cook Lane Surgery is committed to providing a safe, comfortable environment where patients and staff can be confident that best practice is being followed at all times and the safety of everyone is of paramount importance. All of our staff have undertaken formal Chaperone training delivered by the Primary Care Training Centre and have been accredited to carry out these duties.
All patients are entitled to have a ‘chaperone’ present for any consultation, examination or procedure where they feel one is required. The chaperone may be a family member or friend. On occasions you may prefer a formal chaperone to be present, i.e. a trained member of staff.
Wherever possible we would ask you to make this request at the time of booking the appointment so the arrangements can be made and your appointment is not delayed in any way. Where this is not possible we will endeavor to provide a formal chaperone at the time of request. However occasionally it may be necessary to reschedule your appointment.
Your healthcare professional may also require a chaperone to be present for certain consultations in accordance with our chaperone policy. The Clinicians record in your record when a ‘chaperone’ has been offered, declined, requested etc and our chaperones also make a separate entry also in to the Electronic Patient Record (EPR).
